Minutes of Meeting with Professor Stephen Nicholls

Thursday 6 July 2017 – 12.30 – 1.30pm

MEETING SUMMARY

Opportunity for the SA ACADEMIC HEALTH SCIENCE & TRANSLATION CENTRE (SAAHSTC) to establish a new trial group.

THEME – GP and hospital connection  for chronic disease management

AIM – Establish improved patient outcomes through a systematic approach to shared care of  discharged patients.

MEASURE OF EFFECTIVENESS – reduction in re- admission to hospital (proposed)

MODEL – Integrated Care Management  made up of medical and health professionals (i.e. GP, specialist, nurse, allied health etc.)

 

AGREED NEXT STEPS

  • Professor Nicholls

    • To gain acceptance from Translation Centre of SAHSTC  (Paddy Phillips and Steve Wesselingh) of establishing a new priority group.

  • Dr R. Pearce & Dr A. Page

    • To identify 3 suitable General Practices to nominate as participants in a trial

 

SPECIAL NOTE

Scope of this trial group to extend beyond cardiology. (Dr Page suggested Cardiac, COPD, Bowel Cancer, IBS and dermatology.   Dr  Pearce & Dr Page to confirm scope of diseases.

Suggested representation on working party comprising of 3 cardiologists (or specialists in a  particular disease), 3 GP’s, 3 respiratory (or specialists in a  particular disease), Nurse practitioner, Specialist nurse, GP Practice nurse, 3 patients, and 3 non-medical.
